Ist eins vielleicht formaler als das andere? Ist das zweite eine Frage, wo man vielleicht wirklich wissen will, wie es jemandem geht? Werden beide vielleicht in unterschiedlichen Regionen benutzt? Oder was auch immer ....
In British English, you'd ask "How are you?" generally, and you'd ask "How are you doing?" to someone who you knew was definitely ill, to ask how their illness was progressing.
It's not the same in US English, but I'll leave that to someone from the US.
