My first association with "swell" is the "Leave It To Beaver" show from the 1950s. It may well have originated earlier but it was at least still active then. I seem to remember Beaver Cleaver's brother used the word a lot, kind of like a trademark. When I was a kid (damals tm) reruns of the TV show were still playing in syndication.
I would only use the word now for comic effect. Or maybe ironically if someone does something really awful. I might say sarcastically, "That's really swell of you!" Or "That's real swell!"
