    Hallo allerseits!

    Mir ist ein einem Arbeitsbuch (von dem irischen Arbeitsamt herausgegeben) folgender Satz(teil) untergekommen:

    ... has to be confirmed by the completion of a pro forma by the CEO.

    Soweit ich weiss wird pro forma nicht als Nomen verwendet (siehe auch den LEO Eintrag dazu). Es gibt die pro forma Rechnung (pro forma invoice) etc., oder etwas geschieht der Form halber, also pro forma.

    Man kann auch etwas pro forma ausfüllen, aber kann man ein/e Pro Forma ausfüllen?

    Doing something in a pro forma manner is to do it in a perfunctory way to satisfy the minimum requirements or to conform to a convention.

    Sometimes a pro forma or proforma, can refer to a partially completed document, designed by one person to be fully completed and returned by a number of others. The aim being to standardise the information returned to the designer. Usually a pro forma would contain pointers guiding a user in its completion.

    The most common uses of "pro forma" are those that you describe. But the term is also used as a noun, as "shorthand" for "pro forma invoice/statement/questionnaire".

    A Google search suggests it is also being used as a term describing any kind of form or questionnaire. This seems to be especially common on UK government and university sites. I have not encountered this use previously, and Merriam-Webster does not recognize it.
