    What is the best way to translate this? Would you type in (App. 192) to indicate the extension? What is the most common abbreviation for this?

    App. (for Apparat Nummer) is fine, but I'd recommend to give the whole number, like
    01 800 555 5555 192 or +01 800 555 5555 192

    for dummies you may add "Tel.":
    Tel. +01 800 555 5555 192 or
    Tel. +01 800 555 5555 App. 192 is also fine
