Just to kick off (from a complete layman!) - is it possible that this is just figurative and means something like "anaesthetic cover must be considered" i.e., it may be necessary to do the operation under anaesthesia?

Good thought, but it doesn't fit the context of a list of specific requirements for the unit.

2. Anästhesiebogen als Formular

Seems like the writer is just taking pains to avoid confusion with the *anesthesia questionnaire* (which is apparently printed on yellow paper in Germany). The Anästhesiebogen is usually an anethesia elbow (angled bar), not an anesthesia screen or arch.
