first/second = adjective

firstly/secondly = adverb

That seems the best option to me. Fistly sounds fine

Either one is okay, but the adjective is more common in AE. The most important thing is not to mix them. First . . . second . . . OR Firstly . . . secondly . . .
BUT NOT first . . . secondly . . . (or vice versa)

@#3, No. Stick with one form. If you say/write "First of all" then you have to say/write "second", but I would just write first unless I wanted to emphasize the importance or priority of the first statement.
