I checked my Oxford dictionary. It doesn't show it as a verb. Where did you see it?

Merriam-Webster lists "transition" as both a noun and a verb and indicates the verb has been around since 1946.

: to make a transition (transition into college)

Working in education at a high school, I have regularly heard transition used as a verb (and not by the students) with almost precisely this phrase/clause.

Bin auch gerade auf dieses Verb gestoßen.
Je nach Zusammenhang lässt sich vielleicht auch was mit "transient" konstruieren - "be transient", "show transient behaviour" o.ä.

This online dictionary lists transition as a verb too. I'd say it's a recent verb. I remember the days when 'impact' was only a noun and not a verb (at least in BE), and I'm not even that old!

@ 7: Zum Beispiel: transitieren (Astrologie), übergehen, übertreten, überführen. Das hängt, wie immer, nicht zuletzt vom konkreten Kontext ab.

transition as a verb seems to be AE.

Example: he documented, that he transitioned with you on Friday.

In dem Fall heisst es: übergeben, eine Übergabe (von Unterlagen vor einem Urlaub) machen

It's been many years on, and sadly, the misuse of 'transition' as a verb has only gotten worse.... to my ears, it grates like coarse sandpaper on my head - and there is a perfectly good verb to use here - TRANSIT - to transit, it's the real verb. Any dictionary at all will support this idea 🙂
