    = Failure to progress in second stage of labor _____________, relative disproportion

    As far as I can tell, it has something to do with complications during labor, like the infant's head is to large for the mother's cervix.

    Any help would be greatly appreciated.

    That's all there is... but here's another sentence that also uses the term.

    Die Pfeilnaht des Kindes sehr steil im ersten Schrägen Durchmesser, kleine Fontanelle vorne, so dass die Indikation zur sekundären Sectio bei hohem Geradstand gestellt wird.

    =...indicating the need for an emergency c-section 'with high stage longitudinal position'.

    I'm really out to lunch here. The context is a surgical report about the operation of a c-section. Loaded with terms that not even HEXAL knows, like "Hervorlagern". As in "Die Plazenta folgt spontan und vollständig. Hervorlagern des Uterus auf den Bauch. Fassen der Uterotomieecken mit stumpfen Klemmern..." and "stumpfe Erweiterung des subkutanes Fettgewebes".

    1. I misread -- it does say "surgical" report, not "pathological".
    2. I can guess a few things
    This site http://de.wikipedia.org/wiki/Hoher_Geradstand discusses the German terminology. My knowledge of obstetrics is too limited for me to help. It sounds like the Pfeilnaht means the anterior-posterior axis of the baby's head, which is longer than the transverse dimension. It sounds like the baby's head is down but maloriented relative to the axes of the mother's pelvis.
    Hervorlangern sounds something like: The uterus was exteriorized: raised out of the incision, so that it rested on the abdomen (but not excised, presumably it was easier to suture closed), then it was returned to the inside of the mother's belly/pelvis.
    Kleine Fontanelle is presumably the smaller posterior fontanelle (http://en.wikipedia.org/wiki/Fontanelle)

    We are talking about a "hoher Geradstand" when the Head of the fetus is not in the right position at the entrance to the pelvis. Normally the head is turned in a way so that the eyes of the baby look to the left or to the right side of the mother and the shape of the head fits perfectly to the shape of the mothers pelvis. Your baby obviously looked with its eyes to the belly (or the back) of the mother and this leads to a prolonged labour and often to a cecarean section.

    Birth is being divided into the first stage, when the cervix is dilationg ( = Eröffnungsperiode) and the second stage of labour (Austreibungsperiode)
    This means, that the cervic was being totally dilated when the diagnosis was made.

    "hoher Geradstand" means a complication at the beginning of the second stage of labor. The fetal head needs to "look" sideways to enter the pelvis. If not, the head stands "strait" (gerade) in the entrance (hoch) of the pelvis. After 2 hours, a c-section is indicated.


    While the german terminus do not differ wheather the face is looking backwards or forewards, the english terminus "occiput posterior position" describe the situation when the fetal face is in front - which is the most common situation in this complication.


    The international classification of diseases (ICD 11) term JB04 (or older system ICD 10 O46.0) summarize several types of malpositions and malpresentations of the fetus, so the term might be less specific.
