    Dafür gibt es einen Fachausdruck, der mir allerdings nicht mehr geläufig ist. Er wurde in einer langen Diskussion über "Ärztliche Schweigepflicht" und "Arztgeheimnis" erwähnt. Vielleicht findest Du etwas im Archiv.

    "authorisation/authorization for release of medical information"
    "authorisation/authorization for release of medical records"
    "authorisation/authorization for disclosure of medical information/records"
    or
    "consent for ..." instead of "authoris/zation for".
