2. I can guess a few things

It sounds like the Pfeilnaht means the anterior-posterior axis of the baby's head, which is longer than the transverse dimension. It sounds like the baby's head is down but maloriented relative to the axes of the mother's pelvis.
Hervorlangern sounds something like: The uterus was exteriorized: raised out of the incision, so that it rested on the abdomen (but not excised, presumably it was easier to suture closed), then it was returned to the inside of the mother's belly/pelvis.
Kleine Fontanelle is presumably the smaller posterior fontanelle (
